You can ask us to delete the personal information ServEase holds about you. There is no form to fill in and no account to sign into — send one email.
We will confirm we have received it within 7 days, and complete the deletion within 30 days. We will write to you again when it is done.
We may ask you to verify your identity first. Not to create obstacles — to make sure nobody else can have your medical records deleted by pretending to be you.

Where anything is retained, we will tell you specifically what it is and why, rather than simply declining the request.
The hospital that treated you is the controller of your medical records, so you can also make this request directly to them. Either route works, and we will coordinate with them regardless of which one you choose.
